Emily Johnson is the Community Health Specialist at YCCO. She serves as a go-between from the YCCO health plan to community needs.

“My work involves joining together a Community Advisory Council (CAC), made up of OHP Members and community folks, to advise YCCO on issues with the health plan, as well as barriers or concerns within the community. We oversee a Community Health Improvement Plan (CHIP), which includes social needs like housing and food security, health needs like access to mental health services, prevention needs like reproductive health, and equity needs like language access.

I love connecting with people and leading program work that helps prevent health problems before they happen or mitigate them when they do. Sometimes I get to award grant money, which is a lot of fun.”

Emily has been with YCCO for over seven years; here is her perspective on why she loves working for Yamhill Community Care.

“YCCO staff genuinely care about this community, and most of us live in the area, so there’s a personal investment in what we do. We are doing cool, innovative things, and it’s been amazing to watch the company grow.”

When Emily isn’t hard at work, she enjoys being outdoors, in whatever way she can, whether she’s hiking to a summit or kayaking or standing on her porch for a 10-minute break of sunshine!

Mina Ornelas is the Health Services Specialist ll at YCCO. Her position is focused on care coordination for members of YCCO.

“I facilitate our Multi-Disciplinary Team meetings (MDT), engaging community partners in case discussions and care coordination. Another part of my role is outreaching to foster parents for our DHS member population to inform and assist with OHA-required assessments. I also review and process all HRS flex fund requests, and recently I took on the task of HEDIS reviews for our prenatal and postpartum care metrics.

I can honestly say that my work is very rewarding as I often have the opportunity to make a positive impact on our members’ lives, and that is priceless to me.”

Mina has been with YCCO for a little over two years; here is her perspective on why she loves working for Yamhill Community Care.

“I love working at YCCO because I am part of a wonderful team; we are focused on providing the best possible care to our members and community.”

When Mina isn’t coordinating care for members, she can be found taking a walk and feeling the wind against her face! She also enjoys drinking a good cup of hot tea.
